Applicant & Confidentiality Agreement *

I agree that for and during the entire term of my employment or serving at Celebration Church that any information regarding Celebration Church participants, including family life, home environment, personal history, mental health issues, finances, or other challenges and obstacles will be kept confidential. This includes information that I observe, read in files or other written documents, or that is shared with me by another staff member, member of the clergy, social worker, or another person. This information shall not be disclosed to another person except someone under the direct authorization of Celebration Church and shall be considered as private and privileged.

I also agree not to exchange personal information such as phone number, address, or email contact information with participants outside of Celebration Church approved activities.
Upon termination or the ending of serve team position, I will continue to treat the information as private and privileged and will not release any such information to any person, firm, corporation, or other entity, by written or verbal statements, except upon the direct written authority of Celebration Church with the written and legal consent of the parent or guardian of any minor or written consent of the participant. Failure to maintain participant information as private and privilege will be considered a breach of confidentiality and will result in disciplinary action up to and termination of employment and/or serve team involvement.
